Health in All Policies (HiAP) initiatives have been touted as a way to tackle social determinants of health by incorporating health equity into decision-making, systems and policies across multiple sectors. However, HiAP initiatives have traditionally been championed and implemented by government agencies, public health or other mainstream institutions that may not recognize inequities embedded in policies, processes and systems. In 2015, the Center for Prevention at Blue Cross and Blue Shield MN embarked on a different approach to fund eight Minnesota community-based organizations to design, lead and implement a HiAP approach with equity as a goal. These organizations, with deep relationships and leadership within their communities, were funded to implement a HiAP approach, identify the agency or institution they would target for change and develop their measures for success. This unique funding initiative put Minnesota communities most impacted by health inequities at the center of driving health equity in policy, systems and environmental change.

We will share key strategies used by the community organizations and examples of their work. Nexus Community Partners will present their work on the City of Minneapolis’ Green Zones Initiative, a project to advance an equity-based framework for environmental justice in Minneapolis neighborhoods most impacted by pollution and environmental degradation. Fairview Health Services Community Advancement will present on Hmong American Farmers Association’s Veggie Rx program, which addressed food insecurity on the East Side of St. Paul by working with Health East to prescribe Community Supported Agriculture (CSA) boxes to patients.

The HiAP funding initiative has not only been transformative for organizations, institutions and communities, HiAP has changed the way that the Center for Prevention funds, how it thinks about working with community and has advanced health equity, resulting in several key policy and systems change successes within Blue Cross.
